I certify that I am the individual with the legal right to control the disposition of the cremated remains of the decedent listed above, in accordance with the provisions of the California Health and Safety Code.

I hereby authorize Memorial Flight, a division of The 111th Group, Inc., to take possession of and scatter the cremated remains by air within sixty (60) days of receipt from the applicant, weather and safety conditions permitting.

I agree to hold harmless and indemnify Memorial Flight, its owners Julie and Pat Belanger, and all affiliated personnel, from any and all claims, demands, or damages that may arise, including those related to the loss of cremated remains due to the actions or negligence of any airline, freight carrier, or mail service involved in transportation.

All reasonable care and due diligence will be taken to safeguard, transport, and scatter the cremated remains in a timely, respectful, and dignified manner. However, Memorial Flight shall not be held responsible for loss, theft, or circumstances beyond its control.

In all cases, the maximum liability of Memorial Flight shall not exceed six hundred ninety-five dollars ($695) for any loss or damage, regardless of cause.

Memorial Flight operates in accordance with applicable California laws and regulations and is solely responsible for the performance of its services. No liability shall be extended to, nor damages sought from, any funeral director or third party in connection with these services.
